Why it is different
Most system design content operates at the level of boxes on a diagram. This operates a layer below, at why a connection pool exists, what actually happens during a TLS handshake, how an index changes the shape of a query plan, and why an unbuffered write behaves the way it does.
Why that layer matters in interviews
It is the difference between saying you would add a cache and being able to say what the cache costs on a write heavy path and what invalidation strategy you would pick. Interviewers push exactly there.
